We welcome you and your caregivers to our office. Thank you for choosing us for your healthcare needs. Please arrive 5-10 minutes prior to appointment. We strive to serve you promptly and on time. Occasionally the doctor may run a little behind, as he may be helping another patient who needs emergency care. As always, you will receive the same attention should the need arise.
To efficiently handle your concerns, please review the following checklist and bring the items that may apply to you:
1. Forms sent by our office to be filled by you
2. Insurance cards
3. Drivers license
4. List of medications, prescription renewals needed
5. List of the issues you want to address at the time of the visit.
Upon arrival, check in at the front desk so that we may give you the appropriate attention. Our office is handicap accessible.
BILLING: We strive to keep the costs of healthcare down by avoiding unnecessary and duplication of tests. Our office will complete and submit all insurance claims so that you are assured to receive the maximum benefits to which you are entitled. Copays are due at the time of appointment. We accept cash or checks only. Patients who disregard bills or neglect to make arrangements to pay bills may be subject to have their overdue bills sent to a collection agency. To avoid such problems we encourage you to work with our billing staff.
REFILLS AND MEDICATIONS Although most prescriptions are faxed to your pharmacy within a few hours, please note that a 24 hour notice is required. Refills may only be issued for those with chronic medical issues if the patient has been seen recently. Medications that require prior authorization take about a week to get processed by the insurance company after the request has been received. It is your responsibility to contact your insurance carrier with questions regarding what medications they cover.
PHONE CALLS A live person answers all phone calls received at the office during clinic hours. We do not use automated machines. To efficiently serve all of our patients, it is appreciated if all the issues can be addressed in one phone call. Our staff will return emergency calls immediately and other calls as soon as possible.
EMERGENCY SITUATIONS If you need to talk with the doctor after hours, please page doctor per instructions at 989-224-2100. At your discretion, you may elect to call 911 and request to be taken to the nearest emergency room.
TEST RESULTS The doctor may order blood and other ancillary testing prior to your appointment. To maximize a satisfactory outcome, we aim to discuss test results, give recommendations for treatment, as well as addressing any questions and/or concerns. Please check with our office if you do not receive the results of pending tests within one month.
COVERAGE Dr. Rajesh occasionally cross covers with other Internists in town.
CANCELLING THE APPOINTMENT If you are unable to keep your appointment please give us 24 hours notice whenever possible. This will allow another patient to be seen.
